  1. Complexity of systems has increased significantly in recent years. Because humans are not good at managing this complexity, a lot of preventable failures happen. Checklists can help reduce these failures drastically
  1. Principles of checklist design:
      • On the checklist, mention what kind of checklist it is. Is it a verbal team checklist? An online team checklist? A solo checklist?
      • Try the checklist multiple times and in multiple settings to discover ambiguities in instructions
      • The checklist shouldn’t distract from the main objective. It should be short, clear, and make the process efficient
      • Have many short checklists instead of a single long checklist. Include what kind of “breakpoints” each checklist should be used for
      • Know if this should be a do-confirm checklist (more of something to check off) or a read-do checklist (more like a recipe)
      • Focus on the steps that are most critical. Include warnings and common mistakes. Everything doesn’t have to be on the checklist. If something is done right all of the time, don’t put it on the checklist
